These terms apply to your use of http://southwestallergyandasthma.com and to the text message program operated by Southwest Allergy and Asthma Clinic, PC (“Southwest Allergy and Asthma,” “we,” “us”).

Text Message Program Terms and Conditions

Program description

Southwest Allergy and Asthma Clinic, PC sends patient service text messages to patients who have consented to receive them: appointment reminders and confirmations, scheduling changes, office notifications, billing and payment notices, requests to contact the office, and care coordination messages. Messages are sent by Southwest Allergy and Asthma Clinic, PC from (435) 688-1128. We do not send marketing or promotional text messages.

How to join

You can opt in any of three ways:

1. On your patient paperwork. Our clinical intake documents include the “SMS / Text Messaging Consent and Opt-In Authorization” form. Check the SMS consent box, initial it, and sign. The box is optional and is never pre-checked.
2. By phone. When you call (435) 688-1128, our phone menu offers to text you at the number you are calling from and reads these terms. Press 1 to agree. If you do not press 1, no text is sent.
3. By texting us first. If you text (435) 688-1128, you will receive an automatic reply describing this program. Reply START to agree. Until you reply START, we send nothing else and staff cannot text with you.

Any way you join, you will receive one confirmation text once you are enrolled. Consent to receive text messages is not a condition of receiving medical treatment.

Message frequency

Message frequency varies, up to 10 messages per month.

Cost

Message and data rates may apply. Southwest Allergy and Asthma does not charge for the messages; your mobile carrier may charge under your plan.

How to opt out

Reply STOP at any time to cancel. You may also reply END, UNSUBSCRIBE, CANCEL, or QUIT. You will receive one confirmation message and no further messages. You may also opt out by calling (435) 688-1128 or emailing southwestallergyandasthma@gmail.com.

Supported carriers

The program is supported by major carriers including AT&T, T-Mobile, and Verizon. Carriers are not liable for delayed or undelivered messages.

Security of text messages

Standard text messaging is not encrypted. Please do not send detailed medical information by text.

Privacy

Your information is handled under our Privacy Policy at https://southwestallergyandasthma.com/privacy-policy/. Mobile information and SMS opt-in data are never sold, shared, rented, released, or traded to third parties or affiliates for marketing or promotional purposes.

Changes

We may update these terms. Material changes will be communicated before they take effect.
