Southwest Allergy and Asthma is currently welcoming new patients. Please contact us with your health or insurance questions, or to schedule an appointment. When calling for an appointment, please be prepared to provide the patient's name, address, phone number, birth date, type of insurance, and reason for appointment.

When you come in for your first visit, you will need to complete forms regarding your medical history and insurance information. Southwest Allergy and Asthma is located at 515 South 300 East Suite #101. We are accross the street from the St. George Temple Visitors Center. A full initial consultation can take anywhere from 15 to 60 minutes, usually about 45-60 minutes.
